Good Faith Estimate & superbills
Every self-pay patient receives a Good Faith Estimate under the No Surprises Act. On request, we provide a superbill you can submit to your plan for possible out-of-network reimbursement. A card on file is required to reserve an appointment. You may be able to use HSA or FSA funds; check eligibility with your plan administrator.
